"Please read the attachments to this Inquiry, in particular the attachment Documentary Requirements. The supplier, by submitting an offer in response to this inquiry, confirms that he is aware that:

1. The proceedings are conducted on the basis of the GTCP in force at Cersanit S.A., the Civil Code and other mandatory provisions of law.

2. For the performance of the concluded Agreement, the GTCP shall apply to the extent not regulated separately by the Parties in the Tender Procedure. The submission of the Offer is tantamount to the acceptance of the GTC by the Supplier.

3. The selection of the Offer will be tantamount to the conclusion of the Agreement between the Parties to the Bidding Procedure. ”We have instructions for suppliers (downloadable from www.opoczno.eu).

EVERY TIME, PLEASE INCLUDE THE REQUIRED ATTACHMENTS TO THE OFFER

- Current extract from the National Court Register or CEiDG or an extract from another official state register of entrepreneurs (in the case of a Supplier registered outside Poland)

- Power of attorney for the Supplier's representative (Appendix No.

The Supplier who did not deliver to Cersanit S.A. also encloses the following documents with the Offer: 1. Certificate of tax residence - in the case of a foreign supplier,

2. Document confirming the Supplier's tax activity,

3. Statement on the Supplier's bank account (8_BANK ACCOUNT STATEMENT)

OFFERS WITHOUT THE REQUIRED DOCUMENTS WILL NOT BE CONSIDERED
